Types of Dental Lasers

Variety of hard tissue lasers and soft tissue lasers are available in dentistry

HARD TISSUE LASER

These type of lasers have wavelength that can be easily absorbed by hydroxyapatite (present in bone and teeth) ,making them more effective for cutting tooth structure . These can precisely cut tooth structure in small amount for shaping process and also for tooth preparations.

USES

Cavity detection- lasers helps to detect caries by reading off byproducts of decay.
Tooth preparation – these can kill and remove bacteria present in the decayed part , thereby eliminating the need of drills and long treatment time. Also lasers can remove caries present beneath the filled part without removing the restoration.
Teeth sensitivity – lasers prevent sensitivity by sealing dentinal tubules that are responsible for hot and cold sensitivity.
Apicoetomy – this procedure is carried out to save the natural tooth by remove root end part of the infected tooth hence , reducing discomfort and recovery period.
Root canal treatment : lasers can remove all the microbes that traditional chemicals fail to remove hence , providing best treatment results.
In pediatric root canal treatment or pulpectomy . Here the laser beam can reach the peri apex or apex of the tooth to remove infection which conventional treatment fails to.
To aid in bleeching or tooth whitening process.
Can help in ridge preparation for placement of complete denture or full teeth set.
SOFT TISSUE LASERS

Soft tissue lasers produce wavelengths that can be easily absorbed by haemoglobin and water. Nd:YAG lasers used in periodontal therapy can effectively kill bacteria and also helps in healing of tissues. CO2 laser used in dentistry can remove tissue in a faster rate and also reduce adjacent tissue damage.

Moreover these act by sealing blood vessels and nerve endings thereby reducing post operative pain. This makes lasers special as they make the procedure comfortable for patients.

USES

Gummy smile correction : lasers can help to expose more tooth surface by reducing gingiva (gum) hence preventing gummy appearance.
In post cancer treatment complication like oral mucositis to help in resynthesis of collagen fibers.
Muscle attachment or frenum removal : helps to treat tongue ties Also done before orthodontic treatment (braces) for those who have midline spacing.
Crown lengthening : inorder to place restorations , gum surface should be reshaped exposing more tooth surface .here lasers can serve the purpose.
Soft tissue growth removal : can aid in painless removal of soft tissue growth.
To remove gum growth over last molar tooth (operculectomy).
To remove gum over tooth for orthodontic needs (to expose canine tooth ).
Can treat and heal ulcers.

Post Laser Therapy

Post operative experience after laser therapy is more likely to be smoother and comfortable for the patient as they provide a painless and hazzle free experience.

Moreover, they do not produce much bleeding as what traditional scalpels produce to carry out a procedure.

Duration Of Treatment

Usually it can take upto an year but this is completely according to the patients case and requirements.

How Invisalign Works

Computer software designs the 3D  plan of the patients case  after scanning of entire oral cavity. It creates a complete treatment plan demonstrating initial position of the tooth to the desired final tooth position.

This gives a clear idea what will be the post treatment effect on patients profile and can motivate the patient to use trays for 22hours a day to achieve their goal faster.

This treatment picture can be used for further future references. The treatment consists of different stages using different sets of trays. Almost 25 to 45 sets of trays are required per patient which is customised as per the requirement. These are very gentle on tooth surface.

First tray is used for 2 weeks and should be worn 20-22 hours a day initially. Subsequent  sets of trays are used as per the changes.

A snug fit is obtained initially when trays are placed which gradually loosens as the teeth are under gentle motion. This gives the first evidence that the change has started occurring in the oral cavity. Also they indicate that next set of aligners should be administered replacing the old one. This procedure is repeated until the final desired results are obtained.

After Care

As these trays are of removable type , they can be efficiently cleaned after every meals. Its very important to ensure that there is no debri on teeth and within the tray while using them as they can affect the force and alter the tooth movements.

After full treatment procedure with all the sets of aligners its necessary to use retainer to maintain the post treatment result.

Instructions

Should wear aligner minimum 22 hours per day.
Remove aligner while having food.
Remove aligner while having hot drinks like coffee and tea.
After brushing clean aligner using soft bristle brush and water.
Start to wear new set of aligner at night.
For proper sitting of aligners use chewies.
For removal of aligners from mouth, use retriever.
It's recommended to remove aligner from inside (palatal / lingual).
Nail filer helps to remove sharp edges from aligner if present.
Minor to moderate pain is quite normal and disappears in few days and reappear when change to new set of aligners.
In case of broken or lost aligner try to change to next aligner if not fitting use previous one and immediately inform the doctor.
Whenever ongoing aligner is removed please keep it in the aligner box. Keep away aligner from sunlight.
Never place aligners in napkins or an other case. Never place in a pocket that can be sat on.
Do not use dettol or denture cleaners to clean aligners or soak them in mouth wash. These products can damage the surface of the aligner, causing it to become dull and more visible.

Fixed Braces

Being one of the fastest growing area in dentistry fixed teeth aligning procedure has evolved into a thriving field over the years.

Fixed Teeth aligning procedure can be done on adult patients with no upper age limit if the health of teeth is good and maintained. This is not only a procedure to change the overall profile, this also enhance patients personality which can further increase the overall confidence.
Leaving skewed teeth intact can further lead to many dental health issues in future like tooth decay, gum diseases and many more. Therefore it is always important to get appropriate treatment at correct time to prevent further dental health issues.
These appliances as the term suggests are fixed forms which cannot be removed as per the will of patient therefore, patient cooperation is the most important factor for the success of the treatment.

Braces are placed on tooth inorder to straighten up or aligne the tooth according to the profile of the patient. Thus they help us to correct irregular tooth position, crowding of teeth, gaps or spaces and also irregular bite position as they are capable of bringing a variety of tooth movements.
The procedure basically involves the use of brackets, bands, and wires. There are different types of braces available starting from the traditional metal braces to tooth colored ceramic braces.

https://shifadentalclinic.in/wp-content/uploads/2022/07/Traditional-Brace-1.jpg

Traditional metal braces

When one hears about braces the picture that suddenly rushes into the mind is of some metal clips on the front surface of tooth connected with metal wires and elastics.Eventhough we have a number of other options available in fixed orthodontic procedure, this form is still popular in the field due to the impact they have in correcting problems or because how well they solve the problem.

https://shifadentalclinic.in/wp-content/uploads/2022/07/Ceramic_braces.jpg

Ceramic braces

They works almost same as that of traditional braces bringing tooth movements in desired direction. Comparatively they add up more into aesthetic side as they mimics the tooth color where traditional gives metallic color. Overall aesthetic view of this form is better than the traditional braces helping to improve the overall confidence and personality of the patient.

https://shifadentalclinic.in/wp-content/uploads/2022/07/Lingual-Brace.jpg

Lingual braces

Working similar to traditional braces lingual braces helps to bring required tooth movement. These are placed behind the tooth thereby serving cosmetic purpose masking the metal color and brackets.
These are of high demand for those patients who are concerned about their aesthetic appearance as the front surface of the teeth will remain intact throughout the procedure.

Care after orthodontic treatment is very important as these tend to collect debris. Therefore it is very important to keep tooth and supporting structures healthy by cleaning them after every meal.

Crowns and Bridges

Dental Crowns

Crowns are permanently fitted caps placed over tooth that has suffered damage or decay. These tooth shaped crowns or caps helps to restore your teeth and smile.
In order to place a crown, existing natural tooth needs to be prepared or shaped according to specified dimensions.
This is followed by taking impression and lab procedures to manufacture crown or cap.

Signs a Crown is Needed
A cracked tooth without symptoms.
The patient experiences sensitivity to bite pressure and temperature, particularly the cold.
Deep decay. (Once the dentist removes the decay, there will not be enough tooth structure left to support a filling.)
The patient who had or in need of Root Canal treatment.
The tooth has a large silver (amalgam) filling that is breaking down.

Dental Bridges

A dental bridge is made with two or more dental crowns on either side of a gap made by a missing tooth, with a false tooth (or teeth) placed in between. These type of fake teeth are called pontics.

Signs patients need a Bridge
The patient is missing one or more teeth.
Patient requiring a fixed option over removable denture.

What are Dental Crowns / Bridges made of ?

All-Metal Crowns

Even though world has shifted to more aesthetic and modern dental restorations , metal crowns are still popular as a posterior tooth restorative choice.

These restorations are made using metal alloys and are known for their great strength and exceptional durability.

The only thing metal crowns lack is aesthetics as they give metal color but can be used to replace posterior most teeth.

Advantages
Stronger and durable compared to other options.
Best choice for patients with bruxism (teeth grinding habit).
More economical.
All-Ceramic

It is tooth-colored and can be customized to match the exact color, texture, and contour of the adjoining teeth.

Also, the material is highly durable and long-lasting.

This is one of the best choice for those patients with aesthetic priorities

Advantages
Biocompatible
Aesthetically pleasing appearance
Stain resistant

Zirconium Crowns

Zirconia crowns are made from a material which is highly biocompatible with human body and exceptionally durable in longevity. These are least likely to cause any type of allergic reactions and can be safely given to those who have metal allergy as the body will not reject it in any circumstance. They are also strong enough to bear and resist forces generated during chewing and other activities. So that they stand as the best option for posterior areas as they take higher load than others.
Also when it comes to anterior region, zirconia stands in the first position as they perfectly mimic the natural tooth with its attractive translucent color which blends well with other teeth in mouth.
Therefore zirconia is preferred by many people over other options.

Procedure Involved in Dental Crown Placement

During the first appointment, tooth to receive crown/Bridge will be prepared/ shaped according to specified dimensions.
This is followed by impression making to register details of tooth for fabrication process.
Our lab then process your tooth details and will accordingly make a crown to satisfy your needs.
Crown fixation is done with the help of dental cement which can fix them permanently over the prepared tooth structure. And this completes the procedure of crown/ bridge fixation.

If you practice healthy habits, proper hygiene and healthy life style, crown and bridges may last for a long time.

Why do I need a crown, and not a filling?

When the tooth sustains an injury that a filling cannot solve, crowns are recommended. Especially after root canal treatment when the tooth structure loses its strength, crowns should be placed to prevent fracture of the tooth.

How long does crown and bridge last?

If you practice healthy habits, proper hygiene and healthy life style, crown and bridges may last for a long time.

What is the difference between crowns and bridges?

A crown is used to restore a single tooth whereas a bridge is used to replace multiple missing teeth.

Can people recognize that I am wearing a crown over my tooth?

Today we have many options available for crowns starting from Metal colored crowns to Natural tooth looking crowns.  Natural tooth looking crowns like zirconia crowns mimics the natural tooth and give a natural appearance.

How are crowns placed on tooth?

Crowns are cemented over the prepared tooth structure with a dental cement which act as a strong adhesive.

How much portion of my tooth should be removed inorder to place a crown?

Usually 1-2 mm of crown portion is removed to prepare tooth for crown placement.

I had a root canal treatment recently and my tooth looks darkened , Can I mask my this dark color with a crown?

When tooth becomes died , the whole tooth structure may turn dark , this is a normal phenomenon. Selecting correct shade crown which matches with the remaining tooth can solve this  problem.

Should I replace my old crown?

If your old crown is damaged, worn out, or if you find any discomfort with it, seek a dental help as soon as possible to rule out the problems associated with it. If there is any damage beyond repair, replacement of crown will be indicated.

Which food items should I avoid while using crowns and bridges?

It is always recommended to avoid biting hard substances like ice, hard candies, raw vegetables and other hard and sticky items with crowns and bridges as they can damage them which may require replacement if it cannot be repaired.

How do I care for my crowns and bridges?

Maintaining good oral hygiene by brushing and flossing twice daily can increase the lifespan of crown and bridges.  Also avoid hard and sticky food that can disturb the restoration.

Implant Supported Overdentures

Implant supported overdentures can be the best alternative  for those patients who finds it difficult to carry a conventional  teeth set.

When conventional teeth set is placed directly on gums, implant supported sets are placed over certain screws that are placed deep inside the jaw bone.

Surgical procedures are carried out to fix these screws in their correct position. Thus these dentures or teeth set take complete support from these screws as the false teeth set have certain slots to connect with them, thus direct connection is obtained.

Patient can confidently use the teeth set while chewing , swallowing, drinking and with all other activities without any risk of loosening or slippage.
Thus this is a permanent stable alternative to traditional one and is mostly recommended for patients with multiple missing teeth.

Advantages

  • Helps to reastablish chewing efficiency
  • Aesthetics (looks like natural teeth)
  • Improves biting capacity

Rootcanal treatment

Root canal treatment is a dental procedure that relieves pain caused by an infected tooth. Root canal treatment is designed to eliminate bacteria from the infected root canal, prevent reinfection of the tooth and save the natural tooth. When one undergoes a root canal, the inflamed or infected pulp is removed and the inside of the tooth is carefully cleaned and disinfected, then filled and sealed.

Symptoms that indicate need of root canal treatment
  1. Severe pain while chewing or biting
  2. Pimples on the gums
  3. A chipped or cracked tooth
  4. Lingering sensitivity to hot or cold, even after the sensation has been removed
  5. Swollen or tender gums
  6. Deep decay or darkening of the gums
https://shifadentalclinic.in/wp-content/uploads/2020/01/rootcanal_treatment_img.jpg
Saving the natural tooth with root canal treatment has many advantages:
  1. Efficient chewing
  2. Normal biting force and sensation
  3. Natural appearance
  4. Protects other teeth from excessive wear or strain.
Single-visit RCT

Single-visit endodontic treatment is faster, well accepted by patients and prevents the recontamination of root canals between appointments.Single-visit RCT involves cleaning, shaping, and obturation during a single treatment session.

Multiple visit RCT

Multiple visit RCT has been divided into two or more appointments to disinfect the canal, improve patient comfort and observe healing before permanent filling.Multiple-visit treatment includes placement of a short-term medicament, such as calcium hydroxide, for the time between the cleaning and shaping appointment and the obturation appointment.

Rotary endodontics

Instead of using traditional manual files and stainless-steel instruments to treat a root canal, Rotary endodontics employs electrically-powered instruments to perform treatment on root canals. The hand tool we use is equipped with a tip made of nickel titanium. This material is 5 times more flexible than stainless steel.

Due to the flexibility, the nickel-titanium instrument is able to perform unique movements that allow the tool to insert deeply into a curved root canal without damaging or stretching the passageway. This allows a more reliable procedure to be done in a quicker manner.

Dental Implants

The art of dental implant enables metal titanium post to get implanted surgically into the jaw to support a prosthetic tooth.  Implants also hold dentures and dental bridges in position.

Implants are mostly prefered as they resemble nature teeth in appearence and feel.

Implant crowns

Implant crowns are the finishing touches after dental implants have been installed. These devices are placed on the abutment fitted on the external facing end of the implant and they look just like real teeth.

Implant supported overdentures.

These are the kind of removable dentures connected to dental implants. The implants are fitted with a special mechanism that clicks with the dentures to lock them securely.

Basal implants

Biocortical implantology is a modern system which utilises the basal cortical portion of the jaw bone for the retention of the dental implants. This helps in the rehabilitation of partially or completely edentulous patients.

Tooth Decay and Fillings

Decays and cavities are the most common health problem affecting human race.

Anyone with a tooth can get caries. Almost all individuals in their lifetime would have experienced a tooth decay . Some might be painful, however painless cases are also found. Simple filling should be done at the earliest to prevent penetration of caries to deeper structures. But non restorable and untreated cases might lead to root canal treatment.

Causes of tooth decay:

  • Plaque(soft deposit ) on tooth. These are sticky in nature and results as a result of frequent consumption of sugar and starch and without cleaning tooth in regular intervals.
  • Dry mouth
  • Bedtime infant feeding
  • Inadequate brushing and flossing
  • Eating disorders
  • Location of tooth

Cavities are permanent thus if they are left untreated they can eventually increase in size. So that its always better to treat before they enlarge in size as the hole or cavity can initiate a good bacterial communication with the teeth.

As most of the cases are completely asymptomatic its always better to get regular dental checkups done to terminate the progression of caries if detected. Regular dental checkups, cleaning, and flossing can prevent caries formation to a greater extent.

Dental Fillings

Fillings may be permanent or temporary. Temporary fillings are those which are placed for a week inorder to monitor those tooth which have potential chance of infection. They are semipermanent and areint meant to last long. If it stays asymptomatic for a week they are usually replaced with permanent one whereas any unpleasant symptoms suggest immediate removal of the filling
followed by root canal treatment or tooth removal. These are selected mostly according to the condition of tooth of the tooth and extent of decay.

Most common type of fillings are tooth colored composite fillings and glass ionomer fillings (these release fluoride and are most commonly used to fill decay below gum line).

Both of these are biocompatible and donot cause any irritation or infections to the tooth and
its surrounding structures. And also have the capability to arrest further decay by
completely filling the hole or cavity.

If proper care is given after filling , they may last unaffected for a very long time without any symptoms. Sensitivity and pain can be thus resolved by filling the tooth as soon as the cavity is noticed.
